COVID VACCINE NOTICE
National Guidance on Vaccinations
The Government has set out that appointments for a second dose of the COVID-19 vaccine will be brought forward from 12 to 8 weeks for the remaining people in cohort 10 (people in their 40s) who have yet to receive their second dose.
Patients over the age of 21 can book their vaccination appointments through the National Booking System (https://www.nhs.uk/conditions/coronavirus-covid-19/coronavirus-vaccination/book-coronavirus-vaccination/).
Unfortunately the surgery is not able to assist with queries regarding the national booking service.
In line with JCVI guidance, the National Booking Service now allows people in this age group, without an underlying health condition, to book into an appointment offering an alternative to the AstraZeneca vaccine.
Pregnant women: the new NBS functionality will enable pregnant women to book appointments at a site that offers the Pfizer-BioNTech or Moderna vaccine following a series of screening questions. This is in line with JCVI guidance and the Green Book.
DATA PROVISION NOTICE – SHARING INFORMATION WITH NHS DIGITAL FOR PLANNING AND RESEARCH
The Practice is required to comply with the Health and Social Care Act 2012. NHS Digital have the power under the Health and Social Care Act 2012 Section 259 (1) to issue a Data Provision Notice.
This mandates us to share information about you unless you tell us not to.
To opt out please complete the Type-One opt out form before the 23rd June 2021 – you can return this to the Practice by email, email@example.com, or hard copy.

HOW TO GET YOUR OFFICIAL NHS COVID VACCINATION STATUS LETTER
COVID-19 vaccination status will now be available for people living in England, for international travel, via the NHS App or by calling 119 to request a letter copy. If you have problems with the app, please use this link to contact them: https://www.nhs.uk/contact-us/nhs-app-contact-us/
Please note the GP practice cannot give you this official notification and we highly recommend using the NHS App for this as it is easily accessible.

The NHS is very busy responding to the coronavirus pandemic and, at certain times, your GP practice may need to prioritise patients with the most urgent health needs.
If you have a health issue that is not urgent, advice is available on the NHS website www.nhs.uk or NHS app or you can speak to your local pharmacist. You can also use e-consult which is available through our practice website www.bathroadsurgery.co.uk. If you are not sure what to do, please contact the practice who can advise you on how to get the help you need.
Please continue to attend tests, vaccinations and screening when invited.
www.111.nhs.uk is also available 24/7 for urgent health advice or by calling 111, and 999 is for emergencies.
If your child has COVID-19 symptoms, keep home from school.
Dial 119 or use www.gov.uk/coronavirus for a COVID test. Self-isolate until you have the results.
If no test is possible, isolate as per government guidelines.
GPs/A&E can’t provide COVID testing or a letter to allow return to school.
Please note that people in the 50-64-year old age group will not be vaccinated until November and December, providing there is sufficient vaccine, and no appointments will be offered for this age group until then. This is to ensure that those who are most at risk are vaccinated first. If you are between 50 and 64 and you are in one of the other groups which is eligible for the flu vaccination, for example you have a health condition which puts you at risk from the flu, you will be invited earlier.
